LAST WILL AND TESTAMENT OF DANIEL KOWALSKI I, Daniel Kowalski, residing at 137 Beechwood Boulevard, Pittsburgh, Pennsylvania, born March 2, 1982, Pennsylvania driver's license number 31 774 205, being of sound mind and full testamentary capacity and acting of my own free will, declare this to be my Last Will and Testament. 1. We have two children, Emily, born in 2014, and Lucas, born in 2017. I give the remaining one half of my residue, including my PNC Bank accounts and my 401(k) to the extent it passes under this will, to my children Emily and Lucas in equal shares. 5. Guardian. If both parents die while our children are minors, I appoint my sister-in-law, Anna Kowalski, of Mount Lebanon, as guardian of their persons and estates. 6. Substitute heir. If any child does not survive me, that child's share passes to his or her surviving descendants, and if none, to my surviving children equally. 7. Executor. I appoint my wife, Theresa Kowalski, as executor, without bond. If she cannot serve, I appoint my cousin, Robert Nowak, of Bethel Park. 8. Inheritance tax.
